Proof of identity - valid driver's license (Minnesota, or out-of-state) or picture ID.
Proof on residency - valid Minnesota driver license, showing current address, checkbook with current address imprinted on checks, business envelope mailed to the address listed on the application form.
Name and address of parent or guardian is required on the application form if he or she is under 18 years of age.
The library also honors cards from most Minnesota public libraries. You must come to the library to obtain a barcode for your card.
